How to Apply

Apply for Georgia Medicaid

Contact the Division of Family and Children Services (DFCS) to apply for Medicaid at (877) 423-4746.

Request CCSP referral

Call the Georgia Aging & Disability Resource Connection at (866) 552-4464 for a referral.

Complete pre-screening

Answer pre-screening questions to determine preliminary eligibility for CCSP services.

Undergo comprehensive assessment

A case manager conducts an in-home assessment to evaluate your level of care needs.

If eligible, you may be placed on the CCSP waitlist. You'll be contacted when a slot opens.
