CDPAP Explained: New York's Consumer Directed Program

The Consumer Directed Personal Assistance Program (CDPAP) is New York State's self-directed care program that allows Medicaid recipients to hire their own personal assistants — including family members and friends.

Unlike traditional home care agencies where the agency selects and sends an aide, CDPAP puts you in control. You recruit, hire, train, supervise, and if necessary, terminate your personal assistant. This model is especially valuable for people with specific cultural, linguistic, or care preferences.

Eligibility requires active Medicaid enrollment, a need for home care services as determined by a physician, and the ability to self-direct your care (or have a designated representative who can do so on your behalf).

CDPAP allows family members to be paid as caregivers — this includes adult children, parents, siblings, and other relatives. Spouses and designated representatives may also qualify in certain circumstances. This is one of the few programs nationwide that compensates family caregivers.

Pay rates for CDPAP personal assistants in New York typically range from $18.00 to $22.00 per hour, depending on the county and fiscal intermediary. Rates have been increasing following recent legislative changes.

To apply, contact your local Department of Social Services or a CDPAP fiscal intermediary. You will need a physician's order (PRI/SCREEN assessment), proof of Medicaid eligibility, and documentation of your care needs.
